Queensland Health Applications

Online Forms has been replaced by a new website called ERM (Ethical Review Manager) for applications in Queensland. 

ERM will be used to create and submit all applications and post-approval reports for Queensland.

You should be able to log into https://au.forms.ethicalreviewmanager.com with your existing Online Forms email login and password if your submitted application/s have been uploaded to Queensland's Research Ethics Database (RED) systems

Victorian Applications

Alternatively, contact the HREC Coordinator or Research Governance Officer to whom you intend to submit your application; contact details are available at www.health.vic.gov.au/clinicaltrials.

The Victorian Specific Module must be submitted as a supporting document with the NEAF application for any research project involving a site in Victoria. The Victorian Specific Module is available from www.health.vic.gov.au/clinicaltrials



Mater Research Brisbane Applications
