APPLICATIONS WITHIN ACT
For researchers submitting applications within ACT, NEAF applications can now be submitted to ACT Health HREC, check the "NSW or ACT" checkbox on the submission tab to generate a submission code. Further guidance for applications within ACT is available from the Help>Guidance tab.

Researchers submitting SSA forms to a Research Governance Office (RGO) within the New South Wales public health system will no longer need to upload the NEAF supporting documents and NEAF PDF into the SSA. Any electronic documents already uploaded against the NEAF Application Form will automatically be uploaded against the SSA Form ready for import by the RGO. This already applies to VIC, QLD and SA. NOTE - The reviewing ethics committee MUST upload the submission code to their ethics database (RED) in order for the NEAF supporting documents to be linked to the SSA, otherwise the applicant will need to upload the NEAF supporting documents into the SSA Form.

SITE SPECIFIC APPLICATIONS IN SOUTH AUSTRALIA
For researchers submitting site specific applications within South Australia Health, the SSA and LNR SSA forms have been updated with changes to the Insurance and Indemnity section. The applicant must indicate for each of the Associated Investigator(s) if evidence of insurance has been attached.

LNR APPLICATIONS IN NEW SOUTH WALES

For researchers obtaining electronic signatures for the NSW LNR form, electronic authorisation is now available for an investigator, student and Head of Department.

ALL ONLINE FORM USERS

The Transfer rule has change so that transferring project ownership will only require the owner to request the transfer and for the recipient to accept. Recipient of a permanent transfer will have full access to all management functions of the form.

The Create new project rule has changed. Only one ethics form may be submitted per project. To switch to a different jurisdiction or a different form you must uncheck the box to select a different option. A form may not be unchecked if the ethics form and/or associated SSA form(s) have been submitted.

The user manual has been updated (v1.9). If you have viewed the user manual (v1.8) recently, please clear your history/cache before clicking the User Manual link located under Help in the blue menu bar.

For researchers submitting applications within South Australia Health, the name of the low and negligible risk ethics application form has been changed to LNR Ethics SA. This is to avoid any confusion with the LNR SA SSA.

NEAF Update

The NEAF form has been updated with text changes to Section 9.5. This will be available on newly created NEAF forms.

 

New functionality

The multi document upload allows you to upload more than one supporting document on the application's Documents tab. This is compatible with Firefox, Safari, Chrome, Opera and Internet Explorer 10 and above. You can select multiple documents in the one folder and enter the version, document date and description for the selected documents and upload the files at the one time. For more guidance click on the (i) information icon on the documents upload tab.

 

For browsers Internet Explorer 9 or below, you will need to upload documents one at a time.

The NEAF form has been replaced with a new version (v2.2). All new proposals will be created in version 2.2. NEAF was revised to coincide with the release of a revised Chapter 2.3: Qualifying or waiving conditions for consent. Refer to the NHMRC website for details of the changes relating to NEAF v2.2.

The NEAF form has been replaced with a new version (v2.1). You must click on the Manage tab to upgrade your current NEAF application to the new NEAF v 2.1 format. You may wish to take a copy of your current form as not all data will be transferred into the new NEAF form. To do this, generate the PDF and print or save to a local computer drive. Refer to the NHMRC website for details of the changes relating to NEAF v2.1.

The Online Forms website has been updated for users submitting VIC SSA forms to a Research Governance Office (RGO) within Victoria. This includes the following changes:

  • Section 3, Question 8.1 Anticipated start and finish dates at this site for the project is a new question.
  • Section 4, Question 10 Total Funding for the study is now mandatory and must contain numbers only.
  • Section 5, Recruitment of participants is now mandatory and must contain the maximum number of proposed participants to be recruited.

For users using IE 10: When using the Print Draft or Generate PDF feature from Online Forms, the webpage is currently experiencing an issue whereby the page will refresh but the PDF will not be generated. Those users experiencing this issue are advised to use an alternate Internet browser (e.g. Google Chrome) or switch Internet Explorer 10 into compatibility mode (instructions on how to do this are available here: http://windows.microsoft.com/en-AU/internet-explorer/use-compatibility-view#ie=ie-10). This workaround will allow the user to print from Online Forms whilst this issue is being resolved. (Resolved 6 July 2013)

Changes for Victoria and South Australia


From Friday 17 May 2013, researchers submitting SSA forms to a Research Governance Office (RGO) within Victoria and South Australian public health system will no longer need to upload the NEAF supporting documents and NEAF PDF into the SSA. Any electronic documents already uploaded against the NEAF Application Form will automatically be uploaded against the SSA Form ready for import by the RGO.

For Online Forms users submitting SSA forms to a Research Governance Office within New South Wales or Queensland there will be no change, electronic documents from the NEAF appearing in the SSA documents tab are for the applicant only. The applicant will be required to upload NEAF supporting documents and the NEAF PDF into the SSA documents tab if these are required by the RGO.

Online Forms Changes for South Australia
From Saturday 3rd March, researchers submitting applications across the South Australian public health system will be able to generate the following applications from the Online Forms website:
  • A National Ethics Application Form (NEAF); and
  • A Site Specific Assessment (SSA) form.
Researchers are advised, however, to consult the SA Health research ethics web portal for further information on when SA Health HRECs and institutions will be accepting forms generated using Online Forms, www.sahealth.sa.gov.au/researchethics, or contact researchethics@health.sa.gov.au.

For new users, go to the Help tab for information on the SA Health Research Application, or to download a copy of the User manual and to access the How To? electronic tutorials.

For Online Forms users submitting applications in New South Wales, Queensland or Victoria there will be no change regarding the NEAF and SSA Form.

The Online Forms website has been updated to include the Qld LNR (choose the Ethics Form Type 'LNR QLD' from the Project Filter) and LNR SSA Forms (available by clicking on the 'SSA' tab for the Qld LNR).

  • The Site field in the following SSA forms is now a mandatory field. You will be unable to navigate away from this page until the site name has been entered.

CHANGES TO ONLINE FORMS FOR NSW RESEARCHERS

From Friday 22nd July the facility to obtain electronic authorisations for LNR & LNR SSA forms is available.

Note: it is the decision of the relevant HREC Executive Officer and/or the Research Governance Officer as to whether electronic signatures will be accepted and researchers should obtain advice prior to utilising the facility.

The facility is simple to use and can be accessed by going to the Authorisation tab in the form and clicking on Request next to the relevant Authorisation Type. Detailed instructions are available on page 18 of the Online Forms manual, which is accessible from a link in the Authorisation tab, if needed.

In addition some minor enhancements have been made to text and functionality in the system.

IMPORTANT NEW ADVICE RE COMPLETING THE LNR FORM

Please note that after clicking the radio button next to “Is this person the nominated contact person for this project?” you must pause to allow the screen to refresh before proceeding to the next radio button selection; otherwise your next selection will not be saved.

IMPORTANT MESSAGE FOR NSW RESEARCHERS:

From Friday 1st July new versions of the NSW LNR, LNR SSA and Access Request forms (Version 2.0) will replace the old forms.

Applications that have been partially completed on the old forms can be upgraded (auto-populated) into the new format via the Manage tab. Note: there are a small number of fields that will still need to be completed manually after the auto-population.

There are mandatory fields on the new forms which must be completed to enable navigation away from the screen on which they appear. The new versions of the forms (pdf) can be viewed on the Department of Health website.

Applications finalised on the old forms prior to the 1st July will continue to be accepted by Ethics and Governance officers until the end of July 2011.

Researchers should contact the Infonetica Help Desk on 90378404 if they require assistance.

IMPORTANT MESSAGE FOR NSW RESEARCHERS:

New versions of the NSW LNR, LNR SSA and Access Request forms will replace the current forms from 1st July 2011.

From that date you will be required to use the new version of the form for new applications and any that have been partially completed on an old form. It is therefore strongly recommended that you finalise any partially completed applications in Online Forms before 1st July, to alleviate the need for unnecessary re-work.

Applications finalised on the old forms prior to the 1st July will continue to be accepted by Ethics and Governance officers until the end of July 2011.

1. Changes affecting Online Forms users submitting applications in NSW
From Saturday 21st August, researchers submitting applications within NSW Health will be able to generate from the Online Forms website:
• A National Ethics Application Form (NEAF);
• A Site Specific Assessment (SSA) form;
• A Low and Negligible Risk Research (LNR) application form;
• A Site Specific Assessment Form for Low and Negligible Risk Research (LNR SSA); and
• An Access Request form.

For Online Forms users submitting applications in Queensland or Victoria there will be no change regarding the NEAF and SSA Form.

2. Functionality changes affecting ALL Online Forms users
To create a new project, users will click on "Create new project" and three tabs associated with My Project will appear.

My Project tabs:
1. Project Filter tab will ask you to nominate in which State you will be submitting your application and the type of ethics form (i.e. NEAF or, for NSW only, NSW LNR).
2. Transfer tab will enable you to transfer the whole project permanently to someone else. Temporary transfer of an ethics form will continue to appear under the Transfer tab of the NEAF form.
3. Manage tab will enable you to duplicate or delete a project e.g. a NEAF and associated SSA forms (this will replace the Manage tab of the NEAF form).

The printing options have also been simplified:
• You will only be able to select the current page or form for printing.
• You can continue to print with or without text changes.
• You will no longer be able to print the whole application (e.g. NEAF + SSA) and must go to either the NEAF or the SSA to print them individually.

• An Incomplete Page feature to identify which pages of your form you are still working on and pages that are complete
• An added function to the Save this contact to My Contacts icon, now also allows you to update existing contact details directly from the Form
• A personal message box now appears when you Transfer a form to another user or request Authorisation, giving you the choice to include a personal message to the recipient in the auto-generated email notification
• A new column in the SSA documents list identifies documents that have been uploaded from the NEAF

The Online Forms website has been upgraded to improve its user-friendliness. Some key features of the upgrade are:
  • The Home page and main menu at the top of the screen has changed, making it easier for users to navigate directly to their chosen page
  • User manual and How To? electronic tutorials to help users learn how to use the new features, such as Electronic Authorisations which allows for an alternative to ink signatures.
  • New icons to help with typing and editing a form, such as the spell checker, symbols and special characters, text change history and printing drafts in pdf.
  • Many users were unaware of the electronic documents upload function as it was hidden away. The new ‘Documents’ tab is readily accessible, and allows the user to attach any electronic documents to their forms.
  • The Submission feature allows users to create a ‘Submission Code’ instead of a ‘Lock Code’ for their forms. No need to duplicate forms, just make the requested changes to the form and generate a new submission code.

Due to the upgrade, applicants who generated a LOCK CODE prior to the 12th March 2010 and are in the process of submitting their form to the HREC or RGO will need to notify the HREC/RGO of the correct submission code. If required, generate another PDF which will have the correct code in the bottom right hand corner. Click here for step-by-step instructions.
